To take part in the VITA program, please bring identification and tax documents to Salt & Light (1819 Philo Road, Urbana). Without these documents, your tax return cannot be prepared.
You will need to bring:
- Government -issued photo ID for yourself and your spouse. For example, a driver’s license or a passport.
- Your spouse must be present if you plan to file a joint return.
- Social security cards or ITIN letters for yourself, your spouse, and all dependents (for example, your children).
- All tax forms you received such as Form W-2 and Form 1099.
- Form 1098-T if you are a college student. You likely can download this from your student portal.
- Real estate taxes paid and PIN if you own your home.
- Bank routing and account number (if you'd like direct deposit of your refund).
Complete Form 13614-C at home or at Salt & Light
If you’d like, you can download Form 13614-C Intake/Interview and Quality Review Sheet and complete this before you come to Salt & Light. With this completed, you may spend less time at Salt & Light. Otherwise, we will give you a blank form to complete when you check-in.
The Form 13614-C is also available in Spanish and French. Many other languages are available on the IRS (Internal Revenue Service) website; search for Form 13614-C.
