New in 2016, if you were offered health insurance coverage, you will receive the new IRS Form 1095-C, Employer Provided Health Insurance Offer and Coverage. This form is required for your individual tax-filing going forward. Please retain your 1095-C with your W-2 and/or 1042-S when received.
What is Form 1095-C?
Form 1095-C provides information about health insurance coverage, if any, that was offered to you during the previous year. Part I contains information about you and the University of Illinois as an employer. If applicable, Part II contains information about the least expensive option for health insurance coverage that was offered to you during the year, if any – even if you declined the coverage. Part III contains information about any individuals (including you) who were covered under the health insurance plan.

How will I receive Form 1095-C?
Form 1095-C can be delivered to you in one of two ways:
- Electronic delivery is available through the same method as your Form W-2 and/or Form 1042-S. You will need to consent to electronic delivery through NESSIE on the tax statements access page.
- U.S. Mail delivery will be used for your Form 1095-C if you do not consent to receive it electronically. In this case, your Form 1095-C will be mailed to your mailing address on file in NESSIE. To confirm or update your mailing address, visit the Personal Information section of NESSIE and click “Employee Information Form”.
