Coventry Health Care reminds you of the following important information:
- There are no changes to benefits.
- Member ID cards are still valid.
- Continue to call the same toll-free Member Services number listed on member ID cards.
- Change does not affect the list of participating providers.
- Visit www.chcillinois.com (formerly www.PersonalCare.org) for a list of in-network providers and pharmacies, as well as prescription drug lists and other important plan-related documents.
Until all documents transition to the new Coventry Health Care branding, please consider PersonalCare Insurance of Illinois, Inc. and Coventry Health Care to be one in the same. For questions, contact the Member Services number listed on the ID card.
