    FormBuilder Scheduled Maintenance Notice

    Our video credit request system will be unavailable Thursday, June 11, 2026 at 5:00 pm to Saturday, June 13 at 12:00 p.m. CST for AITS on campus to update FormBuilder. If needed, the outage may run longer. During this time frame, you will not be able to submit requests.
