      The links below are to the English and Spanish versions of the updated IFSP pages not in Cornerstone, which are editable or "writable" in Microsoft Word.  Also, please note that Section 7. Implementation and Distribution Authorization language has been updated to accommodate the ability of the team to consult with other IFSP team members more efficiently and without separate, specific consents per provider.  Additionally, the Spanish version has been completed (all pages translated, including the ones in Cornerstone) and for your convenience, pages are presented separately and as one document.

      IFSP Sample Agendas

      This resource page has a "simplified" format and a "detailed" format of a Sample IFSP meeting agenda. Both versions are available as a fillable PDF or an editable Word document.  The simplified version is best for sharing with meeting participants, and the detailed version is best for meeting facilitators (service coordinators).

      IFSP Policy and Procedure in Illinois

      The Illinois Department of Human Services (IDHS) outlines the policy and procedures around Early Intervention IFSPs in Chapter 12 of the CFC Procedure Manual.

      IFSP Development Activities from Provider Handbook (pdf)

      The Illinois Department of Human Services (IDHS) defines IFSP development in Chapter 5.6 of the Illinois EI Provider Handbook.

      This easy-to-read guide takes information from the Illinois EI Provider Handbook and provides clarification on questions that arise due to remote work environments or timekeeping.

      Reading an Individualized Family Service Plan (IFSP)

      The Early Intervention Central Billing Office (EI-CBO) Billing Information for Providers (PDF) provides an overview of the IFSP and its relationship to authorizations and the statewide management information system, Cornerstone.

      In Fall 2021, the Early Childhood Technical Assistance Center (ECTA) and Center for IDEA Early Childhood Data Systems (DaSy) released a Child Outcomes infographic (pdf) that displays the three child outcomes measured by early intervention and early childhood special education systems that span across developmental domains. The infographic is also available in Spanish!

      To help prepare for individualized family services plan (IFSP) meetings, this handout gives an overview of child, family, and functional outcomes, as well as explaining your role in the process and how the outcome measurements are used.

      Criteria Defining High-Quality IFSP Outcomes (pdf)

      This resource from the ECTA Center provides guidelines for evaluating and defining high-quality IFSP outcomes. It is found on page 10 of the Enhancing Recognition of High Quality, Functional IFSP Outcomes training (pdf).

      Created in August 2025, this comprehensive packet has criteria, examples, and resources related to each of the main steps involved in developing an IFSP as a team.  Print version of Developing an IFSP (pdf) is an adapted version for face-to-face sharing, and includes a different coversheet and an intentional blank page for double-sided printing.

      The Early Intervention IFSP Process Flowchart (pdf)

      Document from ECTA that provides detailed steps in the early intervention process, including identification and referral, intake and family assessment, child evaluation and assessment, IFSP development, and service delivery and transition. 

      IFSP-Outcomes Integration Resources

      • Interactive Guide to Implementing an Integrated Child and Family Outcomes and IFSP/IEP Process: From ECTA Center, the purpose of this interactive guide is to assist state, regional, and local early intervention (EI) and early childhood special education (ECSE) providers to take the steps needed to integrate the child and family outcomes measurement with the Individualized Family Service Plan (IFSP) or Individualized Education Program (IEP) process. 
      • Integrating Child and Family Outcomes into the IFSP Process Flowchart (pdf): Similar to the Early Intervention IFSP Process Flowchart, this flowchart shows where opportunities to collect functional information for child outcomes measurement may be integrated throughout the IFSP or IEP process.
      • Integrating Outcomes Learning Community Webinar: This webinar provided an overview of the initial implementation stage, particularly the steps related to implementing an integrated IFSP/IEP and outcomes process. A state representative shared her initial implementation activities and outcomes.  

      Ways To Use Evaluation/Assessment Information In The IFSP Process (pdf)

      A tool from the ECTA Center to help early interventionists use evaluation/assessment information in the IFSP process, keeping in mind who we serve, what we do, where we do it, how we do it, who helps, how often it's done, and how we know it is achieved. 

      IFSP Video: Bri, Her Family, and Early Intervention

      EITP developed an IFSP video that walks a family through the development of the Individualized Family Service Plan. The video is titled "Bri, Her Family, and Early Intervention", and it has been broken down into a series of chapters that highlight the team discussions that occur during IFSP development.

      3 EI Outcomes

      3 Resultados la medición del éxito de la intervención temprana 

      The EI Clearinghouse published this single-page handout for families to help explain the 3 outcomes used to measure the success of EI, which are Family Outcomes, IFSP Functional Outcomes, and Child Outcomes.  This flyer helps explain the key role a parent plays in their child's development and how they can help.
