The purpose of the Child and Family Connections (CFC) Procedure Manual is to assist CFC staff with the procedural operations of the statewide Early Intervention Services System and in the performance of day-to-day activities. Procedures are provided for such activities as referral, intake, eligibility determination, service plan development and monitoring the implementation of service plans.
Related: IDHS for Early Intervention Providers page provides important information regarding updates, reporting and public hearing notices related to CFC Procedure Manual and Part C. Also, the IDHS Publications for Public Awareness page which houses many of the publications used in Early Intervention.
CFC Support Services consist of four subgroups: Social-Emotional Component, Pediatric Consultative Services, Parent Liaison Activities, and Local Interagency Council Coordination.
CFC Listings
Developed by the Early Intervention Training Program (EITP), the intention of this resource package is to further support early intervention professionals’ knowledge and utilization of the federally mandated Child Outcomes Summary (COS) process. This tool was developed and adapted from the Child Outcomes Summary – Team Collaboration (COS-TC) Toolkit. Additionally, many resources from the Early Childhood Technical Assistance Center (ECTA) and The Center for IDEA Early Childhood Data Systems (DaSy) Center are included in this package. Family Outcomes Survey Resources
The Family Outcomes Survey is important to measuring the success of Early Intervention! The following key resources are great to share with CFC staff:
- Illinois Family Outcomes Survey Infographic (pdf) Developed in September 2020, this infographic explains the importance of the survey, the role of providers and service coordinators, and recent updates to survey delivery methodology.
- Illinois Family Outcomes Survey (pdf) Illinois is using a revised version of the original ECO Family Outcomes Survey. Illinois stakeholders were involved in the revision and piloting of the new version.
This Child Find Project Website was created to offer information and resources related to the earliest possible identification of young children and their families who may benefit from early intervention and special education services.

This worksheet is meant to guide the Child & Family Connections (CFCs) and Early Child Care and Education (ECE) Program through the discussion of the MOU agreement. While completing this worksheet will provide assistance in determining the purpose of the MOU, it is important to also read the MOU thoroughly to ensure that both programs understand their roles and responsibilities.
