Description
Service coordinators and other CFC personnel are invited to connect with one another and EITP staff within this forum to discuss successes and challenges that arise while partnering with families during the transition process. Participants can attend as they see fit based on their questions/needs. Before participating, we encourage you to complete a pre-survey to submit questions in advance. See IL SC Forum schedule for times and date.
Objectives
- Learn about ways to ensure smooth and effective transitions for families you serve
- Share strategies, problem-solve, and identify solutions to challenges experienced during the transition process
- Gain a sense of community and learn new ways to partner with families as they transition from/with peers and EITP facilitators
Registration is required
Visit the IL SC Forum schedule to register for the meeting(s) on "Transition Planning Partnerships". Once registered, you will get a confirmation email that contains a pre-survey and Zoom information on how to join the meeting. Registrants are encouraged to submit questions in advance for each forum via the pre-survey, as those pre-submitted questions will be prioritized during the forum.
EI Credit for Participation
1.25 hours in the EI Content Area of Assessment will be awarded to participants.

The following tools were developed by CFC 7 to help quickly determine dates and schedules for the transition of children from EI to ECSE:
- Transition-Discharge Schedule (pdf) - printable schedule to view all birth dates and see the exact dates for when the discharge meeting (120 days) and transition meeting (90 days) are due
- Transition Calculator (Excel file) - enter the child's birthday into the calculator to get the exact dates for meetings from the Transition-Discharge Schedule; a printable schedule is included in this file.
- Transition Timetable (pdf) - visual aid to identify the month when transition needs to begin (i.e., 6 months before the 3rd birthday) for a child based on their birth month
