 The Illinois Department of Public Health and the University of Illinois want to learn more about men's health across Illinois, and we need your help.

  • Statewide Men’s Health Needs Assessment Survey

    This survey is intended for Illinois adults (age 18+) and available to residents and men’s health stakeholders including, but not limited to, those with experience in public health, health care, education, social services, military or veteran’s services, and community-based or outreach services.
